Lace up your walking shoes and join us a for a fun family event, all in support of Mental Health programs at Lakeshore General Hospital! We look forward to seeing you on Saturday, June 4!


  • The event will take place rain or shine!
  • Parking will be available in the lot located on the east side of the DDO Civic Center
  • Please bring your registration confirmation with you (printed or electronic)
  • Bring your own water bottle! Filling stations available on-side in the Civic Center and at the Centennial Park Chalet (halfway point)
  • Bathrooms on-site in the Civic Center and at the Centennial Park Chalet (halfway point)


In consideration of my participation in the Lakeshore General Hospital Foundation 5 km Family Walkathon (further known as the “Event”), on June 4, 2022 at Centennial Park in Dollard-des-Ormeaux, Qc., I agree as follows:

That I knowingly will be participating in an Event that will require me to engage directly with the public. The wearing of a face mask is at my discretion, and is to be provided by me, unless Government rules in effect on June 4th dictate otherwise.

That I will be walking on uneven terrain, and that in the event of personal injury and/or infection by COVID-19 or illness, I will not hold the Lakeshore General Hospital Foundation, their board of director members, employees, volunteers or sponsors responsible for any illness, injury or death as a result from participating in this Event.

ASSUMPTION OF RISK AND RELEASE OF LIABILITY:  Knowing the risks described above, and in voluntary consideration of participating in this Event, I hereby release from and will indemnify, and defend the Lakeshore General Hospital Foundation and their board of directors members, employees, volunteers and sponsors, against any and all claim(s) which I have or may have for any losses, damages or injuries arising out of or in connection with my participation in this Event.

I indicate that I have read the terms and conditions of participating and agree to abide by them. I have carefully read this Release Form and acknowledge that I understand it.  No representation, statements, or inducements, oral or written, apart from the foregoing written statement, have been made. This Release Form shall be governed by the laws of the Province of Quebec. If any portion of this Release Form is held invalid, the rest of the document shall continue in full force and effect.
